Lost your passport? If you’ve checked every part of every room you ever went into, and it’s still not there, you have no choice but to replace it.
Getting a replacement is similar to getting your passport for the very first time. You still use the matching passport application, Form DS-11. Proof of your US citizenship is the most important requirement in replacing your lost passport. Submit your original birth certificate revealing your birth in the US or you can use your original naturalization certificate.
Apart from the passport application Form DS-11, you need to submit the Form DS-64 too. The Form DS-64 explains how you passport was lost.
To begin your application, you will need to visit a Passport Acceptance Agent, because they would need to review your document and verify your US citizenship and identity before they agree for your passport to be replaced.
