Second Passport

The government form one must complete when applying for a second US passport is known as the DS-82.  This is the similar form used when completing an adult passport renewal.  This is a two page form with space for your photo and signature on the first page.  You must include the following information when filling up this application form:

  • Your legal name, as recorded on your current passport
  • Your height, hair color, and eye color
  • Your residential address
  • Your Social Security Number
  • Other legal names you have used, like, maiden names or past marriage names
  • The name and address of somebody who can be contacted if there are emergencies
  • No less than one contact number – home, work, or mobile number
  • The number and issue date your current passport

  • One passport photo – must be a different photo than the one that is on your current active 10-year passport
  • Photocopy of your driver’s license or other state-issued ID (front and back)
  • Your current passport – your full validity (10 year) passport, also called your “primary passport”. If you are renewing a second passport, you should also present the second passport
  • Second passport letter of demand – this letter is addressed to the US Department of State and should be signed by the applicant. The letter must depict the reason you have to get a second passport (either two trips leaving in succession, the second one requiring visas, or upcoming travel to a country that does not allow entry if the tourist has Israel stamps in the passport.)
  • Flight itinerary or other proof of travel showing the trip(s) described in the letter
  • Second passport fee – a check for $110 ($170 if you require expedited service for a second passport) made payable to “US Department of State”
